A Carer’s Assessment
can be provided
if you are 16 years old or over and look
after an adult on a ‘regular and substantial’
basis, you are entitled to a Carer’s
Assessment. This may change with the
latest legislation. Your Carer’s Assessment
will identify your needs as a Carer and help
plan how you can be supported in what
you do.

is a card which

identification in case of accident or sudden
illness. The Council’s Lifeline Service (see

and details of where help can be found for
the person you care for.
