leaves hospital

Before discharge the hospital will
consider your needs when you go
home and if necessary they will
arrange for an assessment of need
from Occupational Therapy, Nursing,
Adult Social Services staff and Age
Connects Morgannwg. Speak to the
Ward Manager or Social Care
Services.
If you have left hospital recently (within 28
days) your local pharmacist (chemist) will
check to ensure that you have all the
medicines you need and that you don’t
take those you no longer need. This is the
discharge medicines review service
and it makes sure that both you and your
GP have the same information about your
medicines after your discharge from
hospital.
