a range of common ailments without
having to make a GP appointment.

Community Pharmacy Discharge
Medicines Review Service
is a free
NHS service for patients (and their
carers) who have left hospital recently
(within 28 days).Your local pharmacist
(chemist) will check to ensure that you
have all the medicines you need and that
you don’t take those you no longer need.
A follow up appointment will check you
have no problems with your medicines.
Take the green copy of the Discharge
Advice Letter you get from hospital with
your medicines to your pharmacist.

Community Pharmacy Medicines
Use Review Service
is a free NHS
service for patients taking 2 or more
prescription medicines for a long term
condition. Your pharmacist will answer
any questions you have about your
medicines and help you manage any
problems you may be having taking
them. Carers can also access this
service with the patient. If you are unable
to visit your local pharmacy, it may be
possible to arrange a review at home.
